Highlighted Programs
Community – based long – term supports and services
Agency programs support local and regional collaborative relationships to promote healthy aging, programming, and choices for frail elders to stay at home. Community based programs such as meals on wheels, bill paying assistance by trained volunteers, medical transportation, and nurse home selection and advocacy are basic to supporting an elder’s chice to remain at home.
In – home choices & support
We offer a wide range of in – home support programs and services such as homemaking, and personal care based on individual needs. The community service plan is set – up, coordinated, and reviewed regularly by our case managers to make sure that it stays current in meeting the elder’s needs and supports his/her choice to stay home.
Care – giver advisor program
Information, referral, and education for caregivers are instrumental in easing the stress of caring for a frail older family member. Our Caregiver Advisor specialist offers advice, information and referral, linkage to resources, support, and/or education for caregivers, and the local business community.
Protective services
Our caseworkers are trained to investigate reports of elder abuse, neglect, financial exploitations, and to work towards a resolution. We work with frail elders whose choice of lifestyles may put them at risk for serious injury or institutionalization, or who are resistant to services, and who would not be able to remain safely in the community without intervention.
